What Happened?According to a notice filed with the Massachusetts AG Office, LHC Group was hit by an unauthorized access incident that began with a voice phishing call to an employee. The breach occurred between April 7, 2026, and April 15, 2026. Information exposed in the breach reportedly included names, addresses, dates of birth, demographic information, clinical summaries, treatment plans, and diagnosis codes. Affected individuals were notified by mail. The work of identifying affected individuals was not begun by LHC Group until July 9, 2026.
