According to a notice filed with the California AG Office, a data breach was reported by Paradigm Healthcare Services, a third-party service provider that processes Medi-Cal billing on behalf of California DHCS. The breach occurred between October 8, 2025 and October 15, 2025. Information exposed in the breach reportedly included full names, dates of birth, gender, and Medi-Cal member identification numbers. Affected individuals were notified by mail beginning September 14, 2026. The unauthorized access to the vendor network was discovered on October 13, 2025, and the review identifying affected individuals was completed on August 18, 2026.
