The breach occurred between October 5 and 10, 2025. Information exposed in the breach reportedly included names, Social Security numbers, dates of birth, addresses, medical information, health insurance information, and scanned government identification. Affected individuals were notified by mail beginning on or around July 20, 2026, and a substitute notice was posted for those whose mailing addresses were not available.
