Center for Urban Community Services Data Breach Class Action Investigation

According to reports filed with the U.S. Department of Health and Human Services Office for Civil Rights, CUCS experienced a network security breach that allowed unauthorized access to their systems. The data breach, which occurred between September 4 and September 10, 2023, resulted in the exposure of confidential patient data including:

  • addresses,
  • Social Security numbers,
  • telephone numbers,
  • dates of birth,
  • benefit identification numbers,
  • health information,
  • and prescription information.

Data breach letters will be sent out by Center for Urban Community Services, informing affected individuals about the nature of the incident and compromised data, particularly those who received services through the New York Department of Health and Human Hygiene and the New York Department of Social Services.
